Physicals are required for all students entering 10th grade (sophomores) and all students new to the District.  This physical examination may be completed only by a Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) before the FIRST DAY OF SCHOOL ATTENDANCE.

Each student's health record should be updated annually by the parent/guardian completing the "Blue Health Form" (Annual Student Health Information).  This form is necessary for student to participate in Physical Education classes.

The district prohibits students from possessing or self-administering medications while on district grounds, on district transportation or during district activities unless explicitly authorized in accordance with this policy.  Medications will only be administered at school when it is not possible or effective for the student to receive the medication at home.

The administration of medications is a nursing activity that must be performed by or under the supervision of a registered professional nurse. A registered professional nurse may delegate the administration of medication to a licensed practical nurse or unlicensed personnel who are trained by the nurse to administer medications. 

The nurse or designee must maintain thorough documentation of all medications administered to students.  Prescription medicine must brought in an appropriately labeled prescription container.  The Policy for Dispensing Medication Form is provided below to submit with prescription medicine for student.

Over-the-counter (OTC) medications 
The district may administer over-the-counter medication to a student upon receipt of a written request and permission to do so by the parent/guardian. All over-the-counter medications must be in the manufacturer's original packaging and will only be administered in accordance with the manufacturer's label. In addition, RNs may administer over-the-counter medications to high school students if a standing order from the district's physician exists and written permission of the parent/guardian is provided on form below.
