Due to Covid-19: 

 

We are asking parents to screen their children each morning. Students and staff should not come to school if they have any of these symptoms:

  • A fever 100.4 or higher, chills or repeated shaking with chills
  • A cough that cannot be attributed to another health condition
  • Shortness of breath or difficulty breathing that cannot be attributed to another health condition
  • A loss of the sense of smell or taste
  • Muscle aches or pains (myalgias) or general fatigue that cannot be attributed to another health condition or that may have been caused by a specific activity, such as physical exercise
  • A sore throat that cannot be attributed to another health condition
  • Congestion or runny nose that cannot be attributed to another health condition
  • A headache that cannot be attributed to another health condition
  • Nausea, vomiting or diarrhea
  • Close contact in the past two weeks with a person who has a suspected or confirmed case of COVID-19

This is required to be updated yearly and can be found on the Parent Portal.  A different form should be completed for each child.

The green paper forms, used in the past are now obsolete.  All RX medication(s) must be in the original container with a current pharmacy label. 

All PRN OTC medication needs to be in the original container, preferably unopened.

 E Forms are required for all medication to be distributed at school.

**Contact us once you have completed the online form to schedule a time for you to bring the medicine(s) to campus. More information is in the attachment. NO MEDICATION WILL BE ACCEPTED UNTIL THIS E FORM IS FILLED OUT/SUBMITTED.

OTC medications: 

Over the counter medications (OTC) can be distributed by the nurse according to district policy, with parent consent via the OTC medication form filled out in the Parent Portal.
